For your best experience, we adhere to certain policies to benefit our valued guests and estheticians, permanent make-up artists and affiliates. APPOINTMENTS RESERVATION, RESCHEDULING, CANCELLATION:
By booking on line or over the phone or in person, you automatically agree to accept OM Spa Polices.
If you want to book outside of regular Om Spa business hours, please let us know in advance and we will try our best to accommodate you.
For your best experience with OM SPA, your safety and treatment's best results, please, check all treatment(s) pre-care/preparation and after care, as well read all Om Spa Policy, BEFORE booking appointment and making payment(s), since the deposits are non refundable. You find all information on this website.
We recommend scheduling your appointments as far in advance as possible to ensure availability.
If you are not sure about your health conditions/contraindication, please, consult your physician to obtain a waiver(s) note.
YOU MUST BE 18 years of age or older to enter our premises, schedule appointment and have a services done. We do not perform cosmetic tattoo on minors even with parent's consent/waiver. Please, seek doctors help.
Om SPA reserve rights to reject a client/or to stop a service at any time, for any reason (usually if we believe that there is some contraindications or your are not good candidate for the treatment)
NO KIDS ARE ALLOWED at at the Spa premises.
ALL SCHEDULING/RESCHEDULING and/or Cancellation is to be made withing through our business phone number 773 853 0839 or via text message to 773 641 3373, where you may leave a message.
RESCHEDULING/OR CANCELLATION for all regular appointments MUST BE DONE WITHING NOT LATE THAN 48 BUSINESS HOURS. If not, you will loose your deposit, and new fee will be required to schedule new appointment.
Appointment(s) scheduled as a MODEL for our training are subject to reschedule or cancel withing not late than 7 business day ($10 cancellation fee will apply).
There is $10 cancellation fee for all appointments, even if they were cancelled withing requested 48 business hours
Every cosmetic tattoo appointment(s) must be booked with $100 non refundable deposit and
all facials services required $25 non refundable deposit; they will be credited towards your service cost.
Deposit(s) can be paid by credit card over the phone,via on line booking on our website www.omspachicago.com, or in person or by certified check signed to Om SPA,LLC and mailed to address: 5910 A West Lawrence Ave, Chicago, Il, 60630.
YOU MUST BE A CARD HOLDER TO MAKE ANY PAYMENTS and YOU MUST BE 18 YRS OLD to book Appointment.
Personal checks are not accepted.
You must present a valid state ID/DL before EVERY tattooing procedure/appointment (including pigment removal, microneedeling/dry tattoo) otherwise we won't perform services; in this case, you will loose your deposit and will be asked to pre pay full service price to book your next appointment.
We do not accept cancellation trough social media, only over our business phone or text message
CHECK IN, LATE ARRIVALS, NO SHOW - NO EXCEPTION
NO SHOW or LATE COMING or CANCELLATION IN LESS THAN 48 BUSINESS HOURS are loosing their deposit and will be charged 100% of service price- NO EXCEPTIONS.
TO BOOK next appointment, you will be asked to pre pay full service fee.
If you don't show up or cancelled to your follow up appointment you are responsible to pay current hourly fee.
Please arrive at least 15 minutes prior to your scheduled appointment for filling paper work (if not filled electronically earlier).
Late arrivals doesn't guaranteed extension of scheduled treatment's time. Treatments will be rendered only for the remainder of the scheduled appointment time and you will be responsible for payment for the full
service and fee for additional services to complete this service will apply.
- NO OBSERVERS/COMPANION in the procedure room.
- NO Food/Drinks are allowed inside of the treatment room.
- NO Drugs/Alcohol/Smoking in premises or outside withing 25 feet
- NO Pets
A brief, complementary consultation is a part of every service/treatment we provide. If you are not sure about your needs or looking for more in depth consultation/analysis, please, schedule additional 30 minutes, paid consultation before coming to your major tretment. At this time we will interview you about your life style, facial features, skin, hair and medical conditions/medications. Suggestions will be made based on your personal beauty goals and budget. The consultation is the ideal moment to ask all questions and get all answers. The fee for this service is not credited towards your major procedure.
We provide consultation in person, over the phone or Skype after scheduling appointment.
A Color Patch Test and /or Skin Sensitivity Patch Test is recommended before attempting cosmetic tattoo/Spa services. It helps to determine healed pigment outcome and client's skin undertone/and potential sensitivity. It is done only upon client's request with usage of products use for treatments. This test is not intended as an allergy test. Consult your doctor if you are prone to an allergies or you already had one, for written consent letter for cosmetic tattoo, BEFORE procedure.
TOUCH UPS- FOLLOW UP, COLOR BOOST
Touch up is the follow up appointment done withing next 8 weeks to perfect first or/and third session.
Color boost is appointment done after 12 months + to refresh faded color;prices will vary from amount of work and time needed.
Usually, it takes at least two to three procedures for every type of cosmetic tattoo/permanent makeup to be consider being completed (except color correction-fixing after other technician work, tattoo lightening and all medical tattooing for skin/scar camouflage, nipple areola, vitiligo, scalp micro pigmentation etc).
ALL SALES ARE FINAL.
NO REFUND OR EXCHANGE POLICY ON ALL OM SPA SERVICES, GIFT CARDS, RETAIL, TRAINING, DEPOSITS.
OM SPA STATES NO MONEY REFUNDS for all provided services, due to the nature and risk involved with specyfice treatments factors and fact, that we are working with human body and that every body reaction to provided service is very unique and unpredictable.

We accept all major credit cards, cash, on line payments and only Om Spa Gift Certificates (we do not participate in third party gift certificates or spa credit cards programs)
We do not accept any medical credit cards nor insurances.
NO PERSONAL CHECKS.
WE DO OFFER IN HOUSE FINANCING PLAN FOR OUR APPRENTICE TRAINING ONLY BUT NOT ACTUAL SERVICES: after paying deposit to secure your training's date, you will decide about weekly, every other week or monthly payments. All payments must be completed 14 business days BEFORE your training start date. There will be $10 additional fee added to every payment for processing credit card transaction and for invoicing service.
IF you booked and pay already for your appointment on line but want to cancel appointment and have your money refund, PLEASE CALL US or E-MAIL to ; don't dispute transaction! We ‘will gladly refund your electronic payment in accordingly amount in reference to our Refund/Cancellation Policy.
You can redeem 100% of OM SPA Gift Card towards your service fee or you can use partially.
NO TIPS ON GIFT CARDS (tips only in cash or on credit cards).
Gift Certificates can be used towards all OM SPA services or skin care purchase.
There is no cash back or cash/money refund over any Gift Certificates.
Gift Certificates can not be redeemable for money/cash value.
All Gift Certificate expires withing 12 months since purchase date.
All promo codes with gift certificates created for special offers have their own expiration date.
All lost/or stolen Gift Certificates won't be replaced.
All Gift Certificates signed up for a person's name can not be transfer be transfer to another person to another
LOST/STOLEN PERSONAL ITEM:
Please leave all jewelry and valuable items at home. OM Spa is not responsible for lost or misplaced items. We regretfully cannot be responsible for any loss, theft or accidental damage to your personal belongings so please take extra caution to prevent any such occurrence. Please do not wear your best clothing for the appointment, as we are working with pigments and clothing can be stained.
